Health Sciences North (HSN) is seeking an innovative leader for the position of Clinical Manager, Northern Outreach & Community Response to lead staff deployment and supervision of the Northern Outreach Team supporting Weenebayko Health Authority (WAHA) and communities, and to coordinate active response of HSN Indigenous navigation support and work with HSN and Indigenous organizations and communities in response to patient, family and community support including emerging areas of escalating risk/community crisis.
The ideal candidate will have a minimum of a Bachelor’s Degree in a health-related discipline, from an accredited college/university. Registration and membership in good standing with a Regulated Health Processional College is required. We are looking for someone with a minimum of 3 (three) years’ experience working with diverse and marginalized population and a minimum of 3 (three) years’ experience working in a leadership/supervisor role preferably in a health care environment. Travel within the North East including to coastal communities at a minimum of annually and self-identification as Indigenous is required.

KEY FUNCTION:
Lead staff deployment and supervision of Northern Outreach Team supporting Weenebayko Health Authority (WAHA) communities. Coordinate active response of HSN Indigenous navigation support and work with HSN and Indigenous organizations and communities in response to patient, family and community support including emerging areas of escalating risk/community crisis.
REPORTING:
Under the general direction of the Director, Indigenous Health.
DUTIES:
- Establish processes to stay informed of Health Human Resource needs of coastal communities in collaboration with leadership from WAHA, where Northern Outreach team support is requested.
- Foster effective communication and relationships with Directors of Care in all WAHA locations and work collaboratively to assign Northern Outreach staff.
- Recruit staff to the Northern Outreach team to meet changing needs; plan staff deployment to coastal communities in collaboration with WAHA leadership (travel, accommodation, assignment) and develop processes that continually monitor staff safety and wellbeing.
- Continually develop Northern Outreach team’s capacity to provide culturally safe clinical care.
- Stay abreast of community health crises in Indigenous communities in the North East (NE) that may require collaborative planning led by Ontario Health (North) and Indigenous community leadership; adjust to rapidly changing conditions, challenges and requirements; mobilize Indigenous navigation services to respond to acute care transfers to HSN as required.
- Support Indigenous engagement activities associated with HSN’s Strategic Plan including health and social service partners together with Indigenous Peoples for the shared purpose of improving health outcomes of Indigenous Peoples.
- Plan, facilitate and participate in Indigenous engagement events throughout the NE and as required on HSN’s Indigenous Health Advisory Council and/or Priority Health Sub Committees.
- Engage in and foster professional development of all staff that promotes cultural safety for Indigenous peoples.
- Act as a resource to other clinical managers, front-line staff and discharge planners at HSN on managing Indigenous patient care issues related to culturally safe care, care transitions and discharge; actively participate in the Clinical Management Committee at HSN.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in the accreditation process and work to ensure that the program and HSN achieve, maintain and continually improve upon their accredited status.
- Teach, coach and lead staff in the development and monitoring of quality indicators and improvement initiatives.
- Engage in and foster professional development of all staff and provide opportunities for learning within budgeted resources.
- Ensure a safe environment for patients, staff and visitors; investigate report, debrief and take or direct corrective action as required on incidents.
- Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Represent the department or program on various committees and in meetings as required.
- Manage human resources which include: recruitment, selection and training.
- Guide, motivate, monitor and supervise staff, develop standards of performance, evaluate performance and make recommendations on disciplinary action as required.
- Evaluate patient experience and satisfaction on an ongoing basis and formulate solutions for improvement.
- Develop annual budgets and assume fiscal accountability of the departmental operation according to approved budget and partnership agreement.
- Provide hospital wide, on-call coverage for problems and serious issues after hours and on weekends according to scheduled rotation.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a Bachelor’s Degree in a health-related discipline, from an accredited college/university.
- Registration with and member in good standing with a Regulated Health Professional College is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of 3 (three) years’ experience working with diverse and marginalized populations, preferably in a health care environment.
- Experience working with remote and northern communities is preferred.
- Minimum of 3 (three) years’ experience working in a leadership/supervisory role, preferably in a health care environment.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of Ontario’s First Nations, Inuit and Métis (FNIM) history, culture, and health/socio-economic issues and needs.
- Demonstrated ability to promote and model the Seven Grandfather Teachings.
- Demonstrated knowledge of FNIM governance structures and protocols, organizations, communities, and geographic territories (e.g. NE region).
- Demonstrated ability to build relationships, foster collaboration, lead transformative change.
- Demonstrated ability to coach, advise and teach others using the principles of adult learning.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated knowledge of relevant legislation and principles of management, hospital committee structure, conflict management and resolution, system needs analysis and organizational assessment.
- Demonstrated ability to use tact and discretion in dealing with health care providers and employees.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated commitment to the safety of co-workers and patients.
