Position Reports To: Director, Stedman Community Hospice & Palliative Care Outreach



JOB SUMMARY

Guided by SJLCB’s Mission, Vision, and Values, the Clinical Navigator is the first point of contact for hospice information and services. Drawing on excellent interpersonal skills and a strong commitment to person- and family- centred care, this role navigates all incoming calls from patients, families, community agencies, and physicians. The Clinical Navigator completes referrals to hospice programming to ensure high-quality, compassionate end of life care in an efficient and seamless manner.


KEY ACCOUNTABILITIES & SUCCESS CRITERIA

Reporting to the Director, Stedman Community Hospice & Palliative Care Outreach and working closely with the broader Hospice team, this role is responsible for:

  • Initial contact for all incoming calls to the Hospice
  • Complete and appropriately distribute new referrals to Hospice programs; closure of files when appropriate
  • Navigate all incoming calls to appropriate internal and external supports and/or resources
  • Education about hospice programs and community resources as appropriate
  • Assist in the prioritization of hospice referrals
  • Liaise with Home and Community Care Support Services (HCCSS), community agencies, clinics, pharmacies, hospitals and physicians as needed
  • Maintain and update all patient medical records/files
  • Collaborate with the Clinical Practice and Learning Specialist to monitor, record and review all Outreach risk indicators and assists in variance reporting to the Outreach team
  • Provide statistical information about Hospice programs to Hospice leaders to support monthly reporting to Finance and HCCSS in addition to the Hospice Foundation, Hospice employees and Hospice Palliative Care Association (HPCO) as requested.
  • Maintain a high level of public relations
  • Maintain an accurate and up-to-date database
  • Drafts bi-weekly reports for all multidisciplinary team meetings/rounds/huddles
  • Participates in Outreach multidisciplinary team meetings/rounds as able
  • Ad hoc duties as assigned

This position may be required to perform additional related job functions beyond this description.

QUALIFICATIONS & SKILLS

  • At least two years working in palliative care and/or community support services
  • Completion of health care education such as Medical Office Administration or Medical/Business Assistant program or equivalent
  • Experience in a customer-focused role and commitment to compassionate person- and family- centered service and care
  • Ability to work autonomously as well as collaboratively with a multidisciplinary team
  • Experience working with local agencies, physicians and clinics
  • Knowledgeable of community support services and resources
  • Excellent written and verbal communication skills
  • Effective and well-developed interpersonal/ customer service skills
  • Demonstrates commitment to diversity, equity, inclusion, and Indigenous Reconciliation
  • Strong computer skills including experience with Microsoft Office
  • Experience using an Electronic Medical Records System (EMR) considered an asset
