NURSE PRACTITIONER
Full-Time or Part-Time, Contract Position
The Upper Grand Family Health Team (UGFHT) offers the opportunity to join an innovative, community-based primary health care team serving the Fergus, Elora and Arthur areas. We are looking for a talented team player to join us as we continue to provide a model of excellence in rural primary health care.
Position Summary
The Nurse Practitioner (NP) is a vital part of the UGFHT and will collaborate with the physicians and other interdisciplinary health care providers to achieve the mission and vision of the organization. The NP will help the UGFHT to meet its strategic objectives by participating in the chronic disease management, health promotion and disease prevention programs.
The Nurse Practitioner reports to the Executive Director. The Nurse Practitioner is responsible for working within the full scope of practice set out by the College of Nurses of Ontario covering the domains of: direct clinical practice, education, and leadership. Key elements of the role include the health assessment and therapeutic management of patients, addressing patient outcomes specifically and globally; patient safety; and quality of care delivery. The Nurse Practitioner will participate in the development of the FHT programs and will work with the multidisciplinary team, patients and community partners to provide integrated services to meet the health needs of individuals, families, groups, communities and populations.
Requirements
- Registered with the current College of Nurses of Ontario
- Certificate of Competence with Primary Care Nurse Practitioner Extended Class certification
- RNAO / NPAO membership preferred
- Masters Nursing Degree preferred
- Valid Ontario Drivers’ License and access to own method of reliable transportation
- Minimum three years demonstrated experience in a primary health care (family medicine) or community setting preferred
- Strong interest and experience in program development, implementation and evaluation of health promotion, disease prevention and chronic disease management programs
- Able to function as a team player working collaboratively with other FHT staff and community partners
- Excellent communication and problem solving skills
- Sound organizational, teaching and leadership skills
- Proficiency in the use of computer and communication system knowledge
- Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
- Preference to be able to prescribe controlled medications
Responsibilities
- Complete health assessments including health history and physical examinations
- Provide wellness care, health screening and care of episodic illness
- Diagnose and treat illnesses and injuries, and prescribe medication in accordance with NP scope of practice
- Screen for the presence of chronic illness and monitor/manage chronic illnesses
- Determine the need for, and order and interpret reports of X-Rays, ECG’s, diagnostic ultrasound and a variety of lab tests
- Monitor the ongoing therapy of clients with chronic illnesses by providing pharmacological and counseling interventions
- Determine the need for, and the implementation of, health promotion and prevention strategies for families and individuals
- Use nursing strategies based on evidence based information
- Formulate and communicate medical diagnoses or disorders
- Consult with physicians in accordance with the standards for physician consultation
- Work closely with team members and Executive Director to monitor and evaluate services and program in order to provide the statistics specific to the work of the NP for the quarterly reports due to the MOHLTC
- Connect with Provincial Nurse Practitioner Association peers in primary care for broad based initiatives and networking
- Participate in self-directed learning to ensure that his/her practice remains relevant by attending professional conferences, e-learning and journal reviews
- Maintain professional competence and remain current (e.g. evidenced-based care) through ongoing participation in professional development, FHT research projects, etc.

- Time management abilities to effectively set priorities and complete tasks within agreed upon timeframes
- Ability to complete lab tests and spirometry, and determine the need to order relevant diagnostic tests
