Aftercare Clinical Lead - Part Time, 20 Hours/week

Wage: $28.84 + commensurate with experience and education

Pine River Institute is a live-in treatment center for youth 13 to 19 years of age struggling with substance abuse and addictive behaviours. Our program includes; a therapeutic wilderness program phase near Algonquin Park, followed by a live-in phase at our campus located on a 200- acre site 100 kms northwest of Toronto. Our program uses a progressive discharge approach that is complemented by an aftercare program. Our family-centered program is innovative and highly structured, and creates opportunities for adolescents to engage in a focused transformation process through a fusion of education, therapy, recreation, and life skills. The majority of our students are from Ontario. We receive funding support from the Ministry of Health and academic support through the Ministry of Education.
Reporting directly to the Off Campus Program Director, the Aftercare Clinical Lead is responsible to support the parents and students as they reintegrate back into their home and their community. The role will liaise with community resources to support the ongoing academic, recreation, volunteer and therapeutic needs of the students. Success in this role will depend on one’s ability to create a supportive culture. The Aftercare Clinical lead ensures that all student, family/caregiver, and community interactions are at a level of excellence.
Key Responsibilities (include, but are not limited to):
- Manage a caseload of 10 to 12 students and support them as they reintegrate into the community. Assist student (with the parent’s help) in finding employment, volunteer positions, therapeutic support, recreation activities, and/or academic placements
- Be a resource to students and their families by creating community partnerships; academic, therapeutic, and volunteer to engage youth and support them with continued growth
- Co-lead weekly psycho-education group on campus for students in the transition phase of the program
- Lead a community Sunday psycho education group 4x/year for parent/caregivers and campus students
- Lead weekly group calls with (1) students and (2) families/caregivers
- Provide individual calls with student and/or family/caregiver as needed
- Coordinate in person student group activity monthly, including alumni students as available
- Liaise with Research Director on follow-up data collection
- Liaise with community resources—academic, therapeutic, recreational, volunteer opportunities, etc.
- Attend and contribute to the weekly Clinical meetings
- On call rotation
Qualifications, Skills & Experience
- BSW or post-secondary degree in relevant field
- Experience with and training in youth care practices in a mental health or youth addiction program
- Experience in outreach services and building community connections considered an asset
- Experience in group psycho educational facilitation
- Ability to communicate effectively with youth, parents/caregivers
- Ability to work with a team of interdisciplinary professionals
- Excellent organizational skills
- Flexible schedule
- Familiar with Toronto and GTA based resources and supports considered an asset
- Valid driver’s license
- Criminal Record/Vulnerable Sector Check
Anticipated time commitment of 2.5 days a week will encompass; one day/week and, 4 Sundays/year at the Mulmur campus, evening hour flexibility for meetings with students, families, and travel to a central location for student in person activity 1x monthly.
