Life Redesign Coaches are based in the homes of the individuals we support where they work collaboratively with others in the delivery of quality people-focused services and the smooth running of a household operation. A Life Redesign Coach is a keeper of the culture of CONNECT and a model for the CONNECT way of being and doing; a ‘go to’ person for residents and colleagues and a partner with others in finding a better way.

Working in partnership with individuals redesigning their lives – at their homes and in the community – Life Redesign Coaches work very closely with the Health and Wellness Team (RNs and LPNs) to provide safe person-centred care. Using coaching methods, they help the people we support set and achieve goals based on what is most meaningful to them. A typical day can include helping with: Getting ready for the day, personal care, getting out into the community, participating in recreation and leisure interests, meal preparation, and, most importantly, helping them reach their individual life redesign goals. The Life Redesign Coaches help carry out the plans informed by the individual and the entire Professional Coaching Resource Team.

EXPECTATIONS

A Life Redesign Coach is expected to:

• Embody CONNECT’s culture
• Support others by ‘doing with, not for’
• Know when to get involved and when to pass
• Contribute to our culture of safety and learning; and
• Consistently meet minimum requirement of 80% coaching competency based on a collaborative peer review process.

KEY TASKS, RESPONSIBILITIES AND COMPETENCIES

Working as a member of CONNECT Partners’ Service Resource Team, you partner with the people we support as they redesign their lives, with a focus on community. We expect you to be:

• Supporting individuals who may be medically frail, have mental health and addictions and who require support in their own homes
• Providing personal care, including (but not limited to): cooking, cleaning, assistance with getting up and going to bed, support with washing, oral care, bathing and dressing, assisting with visits to the toilet or changing incontinence aids
• Medication administration
• Authentic, intuitive, and able to build sustainable, productive relationships with the people we support, their families, and all members of the individual’s service resource team as well as other professionals involved in their care
• Flexible, adaptable, creative, resourceful and resilient, able to competently juggle and manage multiple, sometimes competing priorities while maintaining their sense of perspective and fun
• Willing to walk beside individuals coaching and assisting them to set goals and create a life that is meaningful to them
• Make best use of emergent opportunities for the individual’s real-life learning
• Collaborate with colleagues so the collective efforts equate to one integrated plan that will help create positive new content for the next chapter of the person’s story
• Lean into the expertise of the Health and Wellness Coaches (RNs and LPNs) and work together with them to make lives better for the people we support.
