Education:

Completion of an approved IPAC program of study is preferred, however if not completed Employer sponsored program maybe available.

Diploma in Registered Practical Nursing, as well as a certificate of competence as a Registered Practical Nurse with the College of Nurses of Ontario.

Health and Safety Certification an asset but not required.

Committee involvement:

Infection and Prevention Control Committee chair.

Active member of the Medical Advisory Team along with the Public Health Unit

Active member of the Health and Safety Committee for Occupation Health

Active member of a Regional IPAC group for networking and support

Maintain a system of identifying, reporting and recording infections as per policy, Accreditation Canada standards, MLTC and Ontario Health, Public Health Standards.

Working knowledge of all governing bodies for MLTC, MOL, Accreditation Canada, Ontario Health, Public Health Ontario, IPAC Canada.

Co-ordinate investigation and information management related to infectious outbreaks, surveillance of weekly, monthly, and quarterly infection rates, IPAC auditing for all departments/HH audits with disclosure of findings and teachable moments.

Participate in on-site Outreach services as a team to support outbreak homes. Experience with outbreak management, including COVID19 an asset.

CCC Job Performance

Supervise the appropriate utilization of all resources to ensure appropriate allocation to support required care services; assist the Director of Care develop proposals for the annual operating budget. Ensure the availability of appropriate supplies and equipment at all times.

Ensure the maintenance of a safe environment for residents, staff and visitors; establish and enforce the expectation that hazardous conditions and actions are promptly reported and corrected. Apply the principles and standards of Safety First, using consistency in the application of policies and procedures which have been developed to provide a safe work environment for all staff. Ensure that personal protective devices are provided and appropriately utilized.

In collaboration with the Director of Care, liaise with all other disciplines to coordinate and efficient multidisciplinary system of admission, transfer and discharge which effectively meets the needs of residents, family members, staff and the community. Coordinate the admission interview to introduce the resident to the home and assess his/her future needs and preferences.

Assist the DOC to ensure that documentation in the clinical record is accurate, current and complete. Coordinate and implement activities to ensure that care and funding requirements are met; assist in the facilitation of processes required to ensure that MDS assessments and RAPs are an accurate reflection of the residents and the care that they are provided in the home.

Administer medications as required and provide direct resident care in accordance with the day to day needs of the home.

Be available to work at various times, including days, evenings, nights and weekends as designated by the Director of Care.

Facilitate effective and supportive relationships with representatives of the Ministry of Health and Long Term Care, LHIN, CCAC, Canadian Council on Health Services Accreditation, community partners and other external agencies and organizations.
