The Opportunity:
We are recruiting for a NURSE CLINICIAN RN (FACT)
There are two opportunities available:
- A contract full-time role (35 hours per week Monday to Friday 9am-5pm with flexibility to meet service needs) until November 13, 2023 based at 80 Waterloo Ave. Guelph.
- A contract full-time float role (35 hours per week Monday to Friday 9am to 5pm with flexibility to meet service needs) for approximately one year based at 67 King St. E. Kitchener. This role will float between the various FACT teams in Kitchener/Guelph/Cambridge and the rural Adult Service.
Reporting to the Manager, Adult Intensive Services, this position will provide psychiatric nursing care, mental health assessment, nursing assessments, consultation, and intervention services for adults who experience severe, persistent mental health issues. There will also be an emphasis on education, prevention, and wellness including potential opportunity to provide this information in a group-based format. This position will be a member of a team that will have a flexible approach to providing outreach, intensive, recovery-oriented support and treatment that puts people first. Assertive outreach is part of this nursing role. Collaboration with other disciplines of the FACT team, community service partners, administering injections, managing medications and care planning with the team are an integral part of this role.
Flexible Assertive Community Treatment (FACT) is a recovery-oriented, evidence-based best practice. A FACT team is comprised of a multidisciplinary team of clinicians monitoring and treating clients with severe and persistent mental illness (SPMI). The target client population is those who require both assertive community treatment (ACT) with a larger majority of clients on the team who need less intensive treatment and support and could be served in a traditional intensive case management model. To combine care for these two levels of care, the FACT team employs a flexible switching system where the intensity of services can be ramped up or ramped down as needed depending on individual client need. This combination of flexibility and continuity provides better opportunities for recovery and rehabilitation and ties in well with the natural course of SPMI with its recurring episodes and relapses.
Qualifications:
- Degree/Diploma from an approved Registered Nursing Program
- Current registration with the College of Nurses of Ontario and eligibility to practice as a Registered Nurse in Ontario
- 3 to 5 years of related experience in providing community-based mental health services
- Thorough knowledge of psychiatric nursing and medications
- Well-developed assessment and intervention skills
- Good organizational, interpersonal, and problem-solving skills
- Demonstrated understanding and sensitivity to social and cultural diversity
- Ability to work collaboratively in a fast-paced, outreach settings and multi-disciplinary teamwork
- Excellent verbal and written communication skills
- Proficient with Microsoft Office, Outlook and various database software
- A valid driver’s license, vehicle and insurance, as travel is required

CLASSIFICATION AND RATE OF PAY: Level 6 (OPSEU) - $34.28 to $41.78 per hour
